What is Aftercare?

Cameron Hill Aftercare is a supportive service that provides one-on-one training to young adults transitioning from foster care. It is our goal to assist each young adult with developing and implementing their individualized goals and plans in an effort to help them reach self-sufficiency. We are a hand-up not a hand-out program. We recognize that each young adult’s situation is unique in nature; therefore we will strive to establish creative ways to ensure that the young adult can be successful in reaching their goals.

Who is eligible for aftercare?

Young adults who are 17 ½ years old who were in care at least one day after the age of 16.

How Long can a young adult participate in aftercare?

Aftercare services are available until the young adult’s 21st birthday.

How are you referred to aftercare?

You may be referred to aftercare through a social worker, probation officer, or self referral.

How are sevices accessed?

Once a young adult is referred to the aftercare program an intake worker collaborates with the young adult to complete an intake and to develop goals and plans.  Upon completion of the intake process an aftercare worker is assigned to the young adult.  The aftercare worker assists the young adult with accomplishing goals and plans via telephone contact, direct contact for one-on-one training, and connecting young adults with resources available in the community.  It is the goal of the aftercare worker to facilitate the young adult’s efforts toward self-sufficiency.  The young adult is encouraged to stay in contact with their aftercare worker, and to actively participate in the accomplishing of their plans.
